Öğe Assesment of health system performance in Türkiye over 2000-2023: A descriptive international comparative analysis(İstanbul Ticaret Üniversitesi, 2025) Boyacı, İsrafil; Ahmed, RimonThis article aims to provide an assessment of the Turkish health system, which has been reformed with the Health Transformation Program 2003-2013 (HTP). The comparative performance of the transformed Turkish health system has been descriptively investigated using a sample of country-level data covering the years 2000-2023. The analyses, which contain both developed and emerging countries, reveal the level of progress and trends across the globe in the commonly used indicators about healthcare expenditure, health insurance coverage, healthcare inputs, healthcare utilization, health outcomes, and risk factors. As a result of the evaluations in this article, it has been revealed that despite lower healthcare spending, Türkiye has significantly improved the population health outcomes with relatively less healthcare spending, particularly during the HTP reform period; however, continuity of this progress has not been observed after the reform period. The findings of the article call for more investigations to address the question of whether the Turkish health system has spent less on healthcare over the last twenty years while getting more better health outcomes. Thus, this policy-relevant article points out the importance of continued investment to healthcare as well as the efficiency and productivity issues in Türkiye's healthcare system as topics that need to be researched further.Öğe Dönüşen Türkiye sağlık sisteminde özel hastanelerin yayılımı(2024) Boyacı, İsrafilBu makale, Sağlıkta Dönüşüm Programı (2003-2013) altında gerçekleştirilen sağlık reformlarıyla dönüşen Türkiye sağlık sisteminde özel hastanelerin büyümesini ve ülke çapında yayılmasını ortaya koymayı amaçlamaktadır. Makalede, ülke sağlık sisteminin hastane kaynağının geçirdiği dönüşüm açıklığa kavuşturulmuştur. Özel hastanelerin yayılımına odaklanılarak ülkedeki hastanelerin sektörlere ve illere dağılımlarının yıllar içerisindeki seyirleri, 2002-2022 yıllarını kapsayan veriler yardımıyla betimleyici değerlendirmeler ile incelenmiştir. Türkiye’de hastane sayılarının Sağlıkta Dönüşüm Programı (2003-2013) boyunca yükseldiği, sonraki yıllarda ise yatak sayılarının artışı ile yataklı tedavi kurumları olan hastanelerin büyümelerine devam ettikleri görülmüştür. Dönüşüm Programının son safhalarında yürürlüğe giren kısıtlayıcı düzenlemelerin bir neticesi olarak, özel hastane sektörünün 2010 yılından sonraki büyümesi yeni hastanelerin açılmasından ziyâde, varolan hastanelerin kapasite genişlemeleri ile olmuştur. Ülkedeki özel hastanelerin toplam hastanelerin içindeki payı 2002 yılında %23,4’ten 2014 yılında %36,4’e yükselmiştir ve sonraki yıllarda %37 civarında seyretmiştir. Özel hastane yataklarının ülkedeki toplam hastane yatakları içerisindeki payı ise 2002 yılında %7,5’ten 2014 yılında %19,6’ya yükselmiştir ve takip eden yıllarda %21 civarında olmuştur. Makalede, ülkedeki özel hastanelerin illere dağılımının yıllar içerisindeki seyri de incelenerek ülke sağlık sisteminde gözlemlenen özel hastanelerin yükselişinin coğrafi bölgelere yayılımı da aydınlatılmıştır. Ülkenin kuzeydoğu illerinde özel hastane sayılarındaki artışın nadir olduğu görülmüştür. Doğu, güneydoğu ve kuzey illerinde özel hastanelerin mevcudiyetleri artmış, batı ve güney illerinde ise özel hastane sayıları katlanarak artmıştır. Dönüşen Türkiye sağlık sisteminde özel hastanelerin gelişimlerini ve en nihayetinde ülke sağlık sisteminde oturdukları yeri betimleyen bu çalışmadaki değerlendirmeler neticesinde, esas sorumluluk ve yük kamu sektörü üzerinde olmaya devam ediyor olsa da, Türkiye’de hastane hizmetlerinin sunumunda son yirmi yılda özel mülkiyetli hastanelerin ülke sağlık sistemindeki yerinin genişlediği ve pekiştiği sonucuna varılmıştır.Öğe A cross-sectional survey on the relationship between nurses' fear of COVID-19 infection and burnout: The moderating role of psychological capital(Sage Publications India Pvt Ltd, 2024) Başar, Ufuk; Yalçıntaş, Murat; İyigün, ÖyküThe aims of this research were to uncover whether nurses' fear of catching COVID-19 resulted in burnout, whether their psychological capital remediated the fear of catching COVID-19 and burnout, and whether it moderated the prospective association between fear of catching COVID-19 and burnout. A deductive, explanatory and cross-sectional design has been employed in this research. The sample consisted of 734 nurses from Istanbul, Turkey. A questionnaire survey was conducted to collect data between 14 April and 11 May 2021. Discrepancies between average scores of variables were investigated according to the pandemic-related criteria through t-test and analysis of variance (ANOVA) test. The partial least squares structural equation modelling technique was used to analyse proposed relationships. Findings indicated that nurses from public pandemic hospitals reported the highest levels of burnout and fear of infection and the lowest levels of psychological capital components. Fear of COVID-19 infection resulted in burnout more weakly among nurses who had a high level of psychological resilience. Optimism impacted burnout negatively.
